Building a form

Updated August 5, 2026

Add questions, choose field types, group them into sections, pick a theme, and open or close the form.

You build a form in a short wizard, then fine-tune it any time in the builder. Both ask for the same things.

The create wizard

Creating a form walks you through four steps:

  1. Template — start from a template or from blank.
  2. General — the title (required), a description, optional consent text, the linked event, and a theme.
  3. Fields — add your questions.
  4. Confirm — review a summary, then press Create form.

To change a form later, open it and use the General and Fields tabs of the builder.

Questions and field types

On the Fields step you press Add field and choose its type. The types are:

  • Short text and Long text — free typing, for a name or a longer answer.
  • Email, Phone, Number, and URL — typed answers that are gently checked as people fill them in.
  • Dropdown, Single choice, and Multiple choice — pick from a list you define. Single choice allows one answer, multiple choice allows several.
  • Date — a date picker.
  • Consent — a checkbox with its own legal text above it, for approvals like terms or a privacy notice.
  • Section header — not a question at all, but a heading you drop in to group the questions below it.

Per-question options

Every field has a label — the question people see. Beyond that:

  • Turn Required on to make an answer mandatory. Email and consent fields start as required; you can change any of them.
  • Short and long text fields can carry a placeholder — the faint hint shown inside the box.
  • Dropdown, single-choice, and multiple-choice fields have an options list. Each option has a label and a value; add or remove as many as you need.
  • A consent field has its own legal text, shown above its checkbox.

Reorder your questions with the move up and move down controls on each field.

Theme

From the theme picker you choose how the form looks: Default, Lavender, Ocean, Sunset, Forest, or Mono. Each sets an accent color and overall style.

One thing to keep in mind: the theme shows on the form's own public page. When a form runs inside an event's ticket flow, it appears in a plain style instead.

Open or close it

A form's status is either Active or Passive. Active accepts answers; Passive closes the public link to new answers. If you set a form to Passive while its linked event is still coming up, you'll get a heads-up, since that stops its standalone link.
